The state's child-protection agency failed to respond to serious risks to a Terryville toddler posed by her mother's lengthy history of drug abuse and erratic behavior, and rejected a request by Plymouth police to investigate the family less than a month before 2-year-old Londyn Raine Sack died of an overdose of her mother's prescription drug, the state child advocate has concluded.
