As I read the headline about the failure of Phoenix Sinclair; how the worker neglected to visit the child and ensure safety, I grow angry. This is a senseless tragedy which could have been averted if people were simply performing their duty. This utter failure is reminiscent of my own daughter, Samantha Martin's case.
