LA officials find workers erred in assessing abuse
Los Angeles County child welfare authorities admitted they were wrong to dismiss allegations of abuse against the mother of a 5-year-old boy who was later found brutalized and tortured by her, her boyfriend and his associates in San Bernardino County.
Note: That's what happens when you hire idiots to do a job like this.

They need to suspend or fire workers who take children without providing services first (when possible). It is unfair to suspend or fire worker for failing to protect if they don't suspend workers who damage kids by unreasonably taking children into care.
